Click on your profile icon at the upper right hand corner of any page.

A new popup window will open.

Click on "Default to old layout".

You are now viewing the Care Team Application though the old layout.

Click on your profile icon at the bottom left corner of any page.

Scroll down your profile page to "Default layout" right above the "Save" button.

Click on the down arrow and highlight and select "New Layout".
